All children are required by the Florida Department of Health to have on file in the school office a physical exam (gold form #3040) and immunization record (blue form #680).  Forms are provided by your child’s pediatrician.

 

Ø        A child should not attend school when ill. If your child becomes ill at school, we will attempt to contact you. If you cannot be reached, your child will be cared for in the school office until you arrive.  Please make arrangements for your child to be picked up within as soon as possible.  In an emergency situation, we will call the emergency number(s) you have provided.

Ø        Please call when your child is absent.  Report communicable diseases immediately.

Ø        If medication needs to be given during school hours, the parent or guardian MUST SIGN a medical release form.  Medication must be in the original container and given to the child’s teacher (please do not put medicine in lunch boxes or backpacks).

Ø        If a child is hospitalized, a doctor’s release is required for re-admittance.

 

 

CHILDREN WILL NOT BE ALLOWED TO ATTEND SCHOOL IF THEY HAVE ANY OF THE FOLLOWING SYMPTOMS:

 

Fever: 

auxiliary temperature over 99, oral temperature over 100

Behavior: 

awake all night and crying, unusually tired, pale, irritable or restless

Respiratory:

breathing difficulties

Vomiting: 

more than the usual infant “spitting up”

Diarrhea: 

frequent, watery bowel movements

Rash: 

other than mild diaper rash or heat rash

Sore Throat: 

needs culturing because other signs are present

Runny noses: 

yellowish/green color indicating infection or allergies

Any contagious illnesses or diseases

 

 

 

 

GUIDELINES FOR RE-ADMITTANCE TO CLASS

 

Chickenpox:

all lesions are dry and crusted over

Impetigo:

blisters covered with honey-colored crusts; return 24 hours after the start of medication

Ringworm:

24 to 48 hours after treatment has begun; keep area covered 4 days;                  

Conjunctivitis:

“pink-eye”, redness of eye, burning and thick discharge; return at least 24 hours after start of medication and/or no drainage present;

Lice and Scabies:

return following medical treatment; “NO NIT” policy will be enforced;

Pin Worms:

no restrictions following start of treatment;

Hepatitis:

physician’s statement required;

Strep Throat:

return 24 hours after start of oral medication or 24 hours after injection;

Diarrhea:

NO diarrhea for 24 hours;

Fever:

Temperature not above 98.6 for 24 hours without the use of medication;

Vomiting:

NO vomiting for 24 hours;

Other Contagious Illnesses: 

return when no longer contagious (following doctor’s orders)
